Middle School Immunization Requirements
- Polio IPV – 4 doses at any age, however, 3 doses are enough if at least one was given on or after the 4th birthday.
- DTaP/Tdap – 5 doses, however, 4 doses are enough if at least one was given on or after the 4th birthday. (3 doses meet the requirement if at least one dose of Tdap, DTap,or DTP vaccine was given on or after the 7th birthday.) 1 or 2 doses of Td vaccine given on or after the 7th birthday counts toward the requirement.
- Tdap 1 dose of pertussis containing vaccine is required on or after age 7.
- MMR (Measles/Rubella/Mumps) – 2 doses of measles, 2 doses of mumps, and 1 dose of rubella, separately or combined, given on or after the 1st birthday.
- Varicella – 2 doses are required or an electronic medical exemption from your student's physician documenting a history of Varicella (chickenpox). For complete information about CAIR ME click HERE.A copy of your child's yellow California Immunization Record or other signed medical verification is required upon registration to a district school. Records without your child's first name, last name, full date of birth and the physician's or medical office's stamp or contact information will not be accepted.
High School Immunization Requirements
- Hepatitis B: 3 doses are required. New state immunizations requirements will affect your student progressing from 8th grade to high school. Beginning July 2019, students are required to have 3 doses of Hepatitis B for high school entry.
Medications
An authorization for medication administration must be filled out and signed by both the parent/guardian and the physician in order for the health technician to administer any medications. This includes all over the counter medicine as well as prescription medications. A student may only carry and self-administer an albuterol inhaler or epinephrine auto injector, with a physician's permission on the medical authorization form. Both the medications and the appropriate forms must be brought to the Health Office and logged in by a parent with the Health Tech (NOT SENT IN WITH THE STUDENT) before any will be dispensed. If the required forms are not on file, parents or guardians are permitted to come to school and dispense medication to their child in the Health Office.

PE Exemption
If your child is ill or injured and cannot participate in Physical Education, a signed note from the parent/guardian must be submitted in order to be excused from the class. If the student is going to miss PE for more than 3 days, a physician's note that states a start and end date is required in order to be excused from PE. Again, a signed note is required. No phone calls will be accepted.
